Activation or Reactivation Form
Activate a Replacement wireless device.

Use this form only to request the NWP to activate a Replacement wireless device. Do not submit until you receive your new device.

Reactivate a device that has been suspended or cancelled.

Use this form only to request the NWP to reactivate a device that has been suspended or cancelled. Note: any wireless device that has been cancelled longer than 30 days cannot be reassigned the same wireless number.

Area Code – Number Change Form...
Change an existing phone number to a new number.

Change an existing phone number to a new number, e.g. you want a new area code.

Bulk Order Form...
Order 2 or more devices.

Use this form when placing an order for 2 or more wireless devices. Once the spreadsheet is completed, attach the spreadsheet to the email with the embedded form. Forward email with attachment to 9-NATL-Wireless-Program@faa.gov.

Cancellation Form or Suspension Form...
Cancel wireless Services to a device.

Use this form only to request the NWP to cancel wireless services to a device. (Example: device is no longer needed within an organization or the organization no longer wants to provide funding for the device). Also, when disposing of wireless devices, the old device will remain active until you submit a Cancellation. For more information, see our Disposal & Recycling.

If you order a replacement phone or port your phone number from a different carrier, do NOT submit a Cancellation form. You will keep your existing number. No action is needed on your part. Only submit a Cancellation form if you no longer want to keep your phone number.

Suspend a number temporarily if device is lost or stolen.

Use this form to request the NWP to suspend wireless services to a device if your device is lost or stolen. Suspension only suspends the number which can be added back anytime or after a period of time the number is automatically added back.

Equipment Swap Form...
Move phone number from current device to device on hand.

Use this form only to request the NWP to switch one device to another device you have on hand. (Example: move my phone number from my current device to an extra device we have in the office. The devices must be for the same carrier to complete the swap (Verizon to Verizon, etc.))

Funding Line of Business Change Form...
Change the Funding Line of Business (FLOB) for one or more devices or users.

Use this form to change the FLOB from one FLOB to different FLOB. (Example: An employee is changing orgs and is retaining their GFE. The new FLOB will assume the financial responsibility of any monthly service fees associated with the device in accordance with their IAA.)

International Features (Add or Cancel) Form...
Add/Cancel International coverage to your device.

Use this form only to request the NWP to add or cancel International coverage to your existing device. Please do not use this form to request a new device. If traveling internationally, your Mobility Services Coordinator (MSC) should consult with ASH to see if the country is a "Country of Interest", if it is, a loaner device is required and the MSC should follow their Rules and Responsibilities.

Mass Inventory Change Form...
Request Inventory Change for 2 or more devices.

Use this form to make mass inventory changes to 2 or more devices. Once the spreadsheet is completed, attach the spreadsheet to the email with the embedded form. Forward email with attachment to 9-NATL-Wireless-Program@faa.gov.

New Line Form...
Order a new line.

Use this form only to request the NWP to order a new device from the desired carrier.

New Line of Service on an Existing Device Form...
Assign a new phone number to your existing device.

Use this form only to request the NWP to establish new service on a device you have on hand. (Example: This is a device you have that does not have active service on it and you want to have a new number assigned to it).

Porting Form...
Switch carriers, order a new device from a different carrier, keep your current phone number.

Use this form only to request the NWP to change your wireless service from one cellular phone carrier to another cellular phone carrier, but keep your current cellular number. Porting will require the ordering of a new device. (Example: You currently have the phone number 123-456-7890 with Verizon, but AT&T has better coverage, so you would like to change to AT&T and keep your number 123-456-7890.)

Replacement Form...
Order a replacement for a device from the same carrier.

Use this form only to request the NWP to order a replacement device for an existing user. (Example: currently have device with the carrier, would like a new device with the same number and carrier.)

Troubleshooting Form...
Report an error or issue you are having with your device.

User Name Change Form...
Change the user’s name to a new user for an assigned device.

Use this form only to request the NWP to change the user of an assigned device. (Example: change user from Jon Doe to Jane Smith. Device phone number stays the same. – Device must be reset to factory settings to erase previous user’s personal information and settings)

Voice Mail Password Reset Form...
To reset the voice mail password for your phone.
