Activation Compared to Reactivation
If you ordered a replacement phone or ported your phone number from a different carrier NWP will not activate your device until you request activation. (New lines of service are activated prior to shipment and do not require submission of this form.) If your device was suspended or cancelled and you want to reactivate it utilize the reactivation form. Cancelled devices can only have the phone number reactivated for up to 30 days from date of cancellation.
