Same Location Compared to Multiple Locations
If the bulk devices requested will all be shipping to the same physical location AND are to be added to the same FRAMES cost center select Form 1 – Same location. This will require you to fill in the shipment and FRAMES information once on the web form and will blanket cover all devices listed on the Bulk Order Spread Sheet.

If the bulk devices requested will be shipped to multiple different physical addresses and/or multiple different FRAMES cost centers select Form 2- Multiple Locations. This will allow you to bypass the shipping address and FRAMES information on the web form and you will be required to fill that information out on the Bulk Order Spread Sheet for each of the requested devices.
