Devices not Listed in FRAMES

Devices not listed in FRAMES are transferred using a manual transfer Form 4650-12.

Please complete the form by filling in your Cost Center, Facility Type, Approved By/Title, and Unit Identification. (Note: see the 4650-12 instructional example)

Please ensure the completed transfer document is included in the shipment. Once the shipment is received and physically verified, NWP will accept the transfer and email a signed copy back to you. Enclose a Point of Contact with their telephone number and email in the shipment.

Prior to shipping to the NWP, clear the device and reset it to the original manufacturer’s settings. To reset your device, please refer to the user’s manual as it provides instructions for resetting/wiping the device. If you do not have the user's manual, refer to the carrier’s website. Most carrier websites have the device user manual.

Shipping Apple® Devices?
Please complete the steps below to ensure your Find My iPhone/cellular iPad Activation Lock for Apple devices running iOS 7 or above is deactivated prior to sending your devices to NWP.
Follow these steps:

  1. Go to the setting app on your device
  2. Select General
  3. Select Reset
  4. Select erase all contents and settings.

If device is damaged or will not power on please follow the steps to deactivate remotely.

  1. Sign into your Apple iCloud account: www.iCloud.com/#find
  2. Click the “all Devices” link at the top of the page
  3. Click on the device you would like to deactivate (the device may appear as “offline”)
  4. Click Erase “iPhone”
  5. Click “remove from account” at the bottom of the page
  6. Click “remove” when prompted
